Crown Lengthening

In people with a high smile line, more than 3 mm of the upper gum is visible when smiling despite a normal lip structure. This condition is called a gummy smile. It occurs when the gums cover too much of the teeth.

Gummy Smile Treatment with Laser

In crown lengthening surgery, excess gum tissue is removed with a hard-tissue laser (Er,Cr:YSGG) or a soft-tissue (diode) laser and, where necessary, the bone beneath the gum is reshaped so the teeth appear longer. Crown lengthening also means less gum and more tooth is visible when smiling.

Crown lengthening is also performed to adjust gum levels before restoring teeth that are fractured at the gum line or have decay below the gum line.

Smile Aesthetics

Smile aesthetics has always been important. Aesthetics is subjective, but in dentistry and periodontology smile aesthetics is generally assessed according to a few widely accepted objective criteria. These are:

  • Symmetry of the face and dentition
  • Lip form, lip lines, smile line, tooth proportions, shapes and color
  • Papilla loss and gum defects

The smile line is classified into three types: low, normal and high.

  • Low smile line: less than 20% of the interdental gum is visible. The gum margin is not visible. The teeth are partly hidden.
  • Ideal smile line: 20% to 70% of the interdental gum is visible.
  • High/very high smile line: more than 70% of the interdental gum is visible.
